If you have sideloaded an app, you must let the "Allow installation from unknown sources" stay on "Allow" in the device configuration. If you turn it off, it will automatically uninstall all sideloaded apps on all devices. To make sure it stays installed. Push it as an Enterprise System app as well. That fixed the issue in our similar case where we were not able to upload our app to Private Play Store and needed to sideload the app manually.
