When Agent Cowles first meets John Clancy, the case files she puts down on his kitchen surface are seen in one side shot but have disappeared by the next shot.

When Agent Cowles is at the funeral, the first shot shows no scar on her forehead, but in the second shot, it is there.

The amount of window glass in the train after the shootout is very little considering that all of the windows are shattered.

Charles is hit in the head with a glass and there is a reddish bruise where it left a mark. Shortly afterward, the bruise is completely gone.

As the suspect drops his coffee mug to shoot at FBI agents, the position of the falling mug changes between shots.

Dr. Clancy requests an exhumation and autopsy of one of the murder victims. However, a murder victim would already have been subject to an autopsy given the nature of their death.

When the agents chase the suspect in Yellow Cab 5M43, the suspect pulls out of a line of cabs at a traffic stop. It's unlikely he could have had room to turn his wheels and pull out of the line of vehicles without reversing. They would have been too close to one another.

When the dead woman in the tub filled with flowers is touched by John Clancy, you can see her eyelids move.

The truck that hit the taxi during the chase sequence was driving straight forward from the intersection. However, there was no road in the direction into which it was heading.

As the car chase ends with the car flipping over, it is obvious that a crash dummy is used to replace John in the passenger seat.

When John is walking on the sidewalk, he bumps into a guy. The image of the man hitting his wife flashes into John's mind. The stunt isn't done very well because the guy doesn't even come close to hitting his wife.

When Otis the sniffer dog reaches the corridor, he runs to one of the doors and barks. As the team approaches the door, there is no sign of the dog or the police K9 handler. Also, the dog comes from 'behind' the FBI team as they enter the corridor; who led them to the right floor/corridor?

In one scene, Clancy receives a psychic vision when touching someone while wearing a glove. It seems a psychic who relies on touch would not wear a glove during contact.

Several characters are listed in the credits as members of "Swat" instead of "SWAT."

While John, Joe, and Katherine are in the conference room, the camera pans left to right. The camera is on a cart of some type and it is visible as it is being pushed in front of the half wall.

There is a light or microphone on the ceiling near Jeffrey as he walks into the room with John and Charles. This device is dark colored while the rest of the lighting is sparkling glass and elegant.
